The condition often resembles cancer, infections, or other autoimmune disorders, making early recognition essential to avoid unnecessary surgeries and delays in treatment. What Is IgG4-Related Disease?

IgG4-related disease is a systemic inflammatory disorder characterized by immune cell infiltration, tissue swelling, and fibrosis affecting various organs. The hallmark feature is the presence of IgG4-positive plasma cells within affected tissues, accompanied by a distinctive pattern of scarring.

The disease may involve a single organ or multiple organs simultaneously and often progresses slowly. Because it can form mass-like lesions, IgG4-RD is frequently mistaken for malignancy. Despite involving different organs, the underlying disease mechanism and pathological features remain consistent across all affected sites.

Commonly involved organs include:

  • Pancreas

  • Salivary and lacrimal glands

  • Kidneys

  • Bile ducts and liver

  • Lungs

  • Retroperitoneum

  • Central nervous system

  • Aorta and major blood vessels

Classification Criteria for IgG4-Related Disease

In 2019, the American College of Rheumatology (ACR) and the European League Against Rheumatism (EULAR) introduced standardized classification criteria to support research and clinical consistency.

The framework includes:

  1. Entry criteria – Clinical or imaging findings suggestive of IgG4-RD

  2. Exclusion criteria – Elimination of malignancy, infection, or other autoimmune diseases

  3. Weighted scoring system – Based on pathology, imaging, serum IgG4 levels, and organ involvement

A cumulative score of 20 points or more supports classification as IgG4-RD. While useful for research, clinical judgment remains essential for diagnosis.

First-line therapy for active disease, often producing rapid symptom improvement. Careful tapering is necessary to reduce relapse risk.

Steroid-Sparing Immunosuppressants

Medications such as azathioprine, methotrexate, or mycophenolate help maintain remission and reduce steroid dependence.

Biologic Therapy

Rituximab, a B-cell–targeting agent, is highly effective for relapsing or steroid-resistant disease.

FDA-Approved Targeted Therapy

In April 2025, inebilizumab (Uplizna®) became the first FDA-approved treatment specifically for IgG4-RD, significantly reducing disease flares and steroid reliance.
